We currently accept the insurance plans below. If you do not have one of the plans below, please call our office and we will be happy to assist in any way we can in working with you and your insurance provider.

 

Aetna

     Dr. Levey's Aetna profile

     Specific Aetna plans accepted by Dr. Levey

     If you are an Aetna patient and are at risk or believe you may be at risk for certain cancers with a genetic association such    as breast, ovarian, and colorectal please visit http://www.informedmedicaldecisions.com/. They will further assess your risk    and communicate directly with Dr. Levey.

 

Cigna

     As of January 1, 2009 Cigna will no longer offer HMO / POS to New York patients.

 

Emblem Health

Empire Blue Cross / Blue Shield and any Nationwide BC / BS Plan with PPO

GHI  / Emblem Health (We do not accept GHI HMO. We do accept PPO and CBP.)

     Preffered laboratory provider: Quest 

Healthnet (HMO and PPO only  - not Medicare)

Health Insurance Plan of NY (HIP) / Emblem Health (We do not accept HIP HMO plans- only POS, PPO and EPO)

 

Local 1199 (National Benefit Fund)

     As of 3/31/2008 we do not accept the 1199 Home Care Benefit Fund (group199H).

     We are not part of the Fidelis Care Network. Thus, if you have the Home Care Benefit Fund (group 199H) you will be considered to have 

      out of network insurance and will be charged accordingly.    

Magnacare (PPO)

Medicare  - As of 3/23/2010 we are no longer accepting patients covered by CMS / Medicare. If you are enrolled in a Medicare Advantage plan then we will be able to accept you as a new patient in our practice.
